Who is authorized to complete the I-693 exam?
Only a USCIS-designated Civil Surgeon can complete and sign the I-693 Form. Our clinic has authorized providers who are approved to perform and certify these exams.
What should I bring to my appointment?

A valid government-issued photo ID (passport, driver's license, or green card)

Any vaccination records

A list of your current medications

Interpreter, if needed (must be 18 or older)
What happens during the exam?

A physical examination

A review of your medical history

Tuberculosis (TB) testing (a blood test)

A blood test for syphilis (ages 18-44)

A urine test for gonorrhea (ages 18-24)

Vaccination review and updates (as required by USCIS)
Do I need all my vaccines to complete the form?
No, you only need to start the required vaccinations to receive your completed paperwork. You are not required to finish the entire vaccine series (e.g., for Hepatitis B or MMR) before we submit your I-693.
Can I be seen without having my vaccines done first?
Yes. You can still be seen for your exam without having completed your vaccines. However, you must start the required vaccinations before we can finalize and sign your I-693 form.
Where do I get my vaccines?
Our clinic does not administer vaccines for the I-693 exam. You must contact your local health department to schedule your vaccinations. Click here for a list of local health departments.
How long does it take to receive my completed I-693 form?
Once all required labs and vaccines are complete, your paperwork is usually ready within 7-14 business days. We will notify you when it’s ready for pick-up. You must sign and pick up your OWN paperwork and if you needed an interpreter, they must come with you to also sign or your forms will NOT be valid. This HAS to be the interpreter present for your exam.
How long is the I-693 valid?
As of 2024, a completed I-693 form is valid for 2 years from the date the civil surgeon signs it.
What if I test positive for something?

Your form cannot be completed until the evaluation is cleared
Do you offer same-day appointments or walk-ins?
No, we do not. You must call and schedule an appointment to be seen.
