The Process
The I-693 process refers to the completion of the Immigration Medical Examination and Vaccination Record, which is required for certain applicants applying for lawful permanent residence (a green card) in the United States. This form must be completed by a USCIS-designated civil surgeon after a medical examination to confirm that the applicant is free from health conditions that would make them inadmissible under immigration laws. The medical exam includes a physical examination, a review of medical history, testing for communicable diseases such as tuberculosis and syphilis, and verification of required vaccinations. Once completed, the sealed form must be submitted to USCIS. You can go ahead and fill out the paperwork we require below and bring it to your appointment or you may show up 30 minutes early to ensure the paperwork is completed in time for your appointment.
What To Expect
You must bring a valid ID or Passport and any vaccination records that you may have. They must be translated into English. If you do not speak English, you MUST bring a translator over the age of 18 who is available to return with you to collect your paperwork. You must BOTH sign your paperwork in person. If you need help with that, please email us back and we can provide the contact information for our preferred translator.
The exam costs $450. We complete the physical exam, required blood work, and all necessary paperwork. Please note that vaccinations are not included in this cost and we do not administer vaccines in the office. If you need vaccines, we recommend visiting the Health Department. You are also welcome to use locations such as CVS or Walgreens. However, some patients report that these locations may not administer all vaccines at once. Additionally, certain vaccines (such as the polio vaccine) may require a prescription from your primary care physician, which we cannot provide. If you receive a live MMR or Varicella vaccine, you must wait four (4) weeks before completing your TB blood test, unless the vaccine and blood test are administered on the same day. Receiving a live vaccine prior to TB testing may affect the accuracy of the results and could lead to a false-negative outcome.

We require a $100 deposit at booking to hold your appointment, which will be applied toward your total balance of $450. The $100 deposit is NON-REFUNDABLE.

You can also review the vaccines required by age according to USCIS. Please also complete your paperwork ahead of time using the forms on our website. What to Expect at Your Appointment: During your immigration physical, you will receive a comprehensive medical exam. This includes reviewing your family history, personal medical history, and any medications you are currently taking. As part of the required testing, you will also have blood drawn. You do not have to be fasting. Patients between the ages of 18–24 will be required to provide a urine sample per USCIS guidelines. Please arrive well-hydrated to ensure these tests can be completed smoothly. If a child is scheduled for an exam, it is important that parents or guardians fully prepare them for what to expect, including blood work, and are ready to actively assist in keeping them calm and comfortable throughout the process. To help us provide the safest and most efficient care, we kindly ask that patients—and parents/guardians of minor patients—arrive fully prepared to focus on the exam. While we understand childcare challenges, bringing additional children who do not have scheduled appointments can create distractions and make it difficult to maintain the level of attention required. We respectfully ask that you arrange childcare for any children without appointments so we can ensure a smooth, focused experience for everyone.
Our office usually can get you your paperwork in under 2 weeks, provided your bloodwork tests negative for TB. If the result is positive, you must get a chest X-ray at the Health Department to confirm you do not have an active infection. You must be referred and make an appointment to be seen; this may take a few additional days depending on your schedule and the Health Department's schedule. Please do not panic if you receive a positive result for your TB test, we often receive positive results that turn out to be a latent infection usually caused by tuberculosis vaccines.

The total cost for the I-693 medical exam is $450, and we require a $100 deposit at the time of booking to secure your appointment. This deposit is applied toward your final balance, so you will only pay $350 when you come in for your exam. To complete your I-693 process, you must bring a valid and unexpired government-issued photo ID or passport, along with any vaccination records you have available.
