Required Vaccines

Newborn Baby

Birth-1M (CONTACT OFFICE FIRST FOR CHILDREN UNDER 2 YEARS OLD)

Hep B is all that is required to file paperwork.

If you receive a live MMR or Varicella vaccine, you must wait four (4) weeks before completing your TB blood test, unless the vaccine and blood test are administered on the same day. Receiving a live vaccine prior to TB testing may affect the accuracy of the results and could lead to a false-negative outcome.

2-11M

DTP/DTaP, Polio, Rotavirus (6W-8M Only), Hep B, Pneumococcal (2-59M Only) HIB (2-59M Only), Influenza**  (Seasonal Sep 1st- March 31st) and only if the child is 6weeks or older.

Vaccinations are not provided here—please contact the health department to schedule your appointment. You do not need vaccinations to be seen, but they must be started to receive your paperwork. Completion of the full vaccine series is not required. The TDaP shot must be within the last 10 years.
